Pet Legacy Program

As loving pet guardians, we are sometimes faced with difficult questions like: where will my pet go if something happens to me? At the Humane Society of Utah, we can help you come up with a plan so you have peace of mind when it comes to your beloved animal. When you enroll in the Pet Legacy Program, you will set HSU as the designated location to take your pet if they outlive you. We will provide your companion with the best quality care as we work to find them a loving home. It is our goal to honor the love and compassion that you have for your pet, by providing a safe and caring environment after you are gone. 

To enroll in the Pet Legacy Program, please print and complete the appropriate pet legacy profile(s). With this information, we will be able to match your pet with the best possible fit for a new home! Please use a separate form for each pet, and be as thorough as possible. Be sure to review the program policies and guidelines before enrollment, as there are some limitations to the services we can provide or guarantee.
